On this website you will find information and tools to help with a health behavior change and a database of community programs for health behavior change. Click on a topic to learn more about a health behavior, how it affects your health, strategies for change and other tools. Click on the community resources to find a program in Cuyahoga county that may suit your needs.

What is in our database of community resources?

Educational classes, activities, support groups and hotlines related to smoking, diet/nutrition, exercise and weight management are included in the database. A variety of programs for each topic are included. You can search the database by location, by topic or you may just browse the list of available programs. Clicking on a title of the program will take you to a full-page description, use the back button to return to the list.

Definition of terms used in our database of resources

Title
The descriptive title of the program.
City
The location of the program. Some programs are available in multiple locations.
Fee
Many programs have a fee. Fees are described in more detail on the full description page. In many instances, you will need to call for specific details.
Offered For
Some programs are offered for specific groups, such as seniors, etc.
Bilinugal staff
If a program has bilingual staff available, this has been indicated.
